Dear Friends and Patients,

As we enter the final week in practice at Embrace Wellness, I am filled with many emotions. It has been my privilege to care for so many of you over you over the past 15 years and look forward to continuing to do so at Functional Medicine of Idaho at both their Meridian and Eagle locations beginning in June.

I chose Functional Medicine of Idaho (FMI) and Center for Optimal Health (COH) because I am excited about the expanded offerings and opportunities for the patients that choose to follow. I hope you have heard the exciting news, FMI has decided to waive the membership fee, so if you were hesitating to schedule call today and book your appointment!

If you are interested in scheduling an appointment to see me at FMI or COH, please visit their website, www.funmedidaho.com, click on the “Join Now” icon in the top right corner, fill out the “New Patient Inquiry Form” and in the “Type of Patient” drop down menu, select “Tammy Hadfield Referral”. A scheduling coordinator will then contact you to schedule. You can email [email protected] with any questions about scheduling appointments for June and beyond. Please be advised any currently scheduled appointments after May 30, 2024, will be canceled and a new appointment will need to be made with FMI scheduling.

Please note I will be unable to refill prescriptions for patients who have not established care with FMI or COH after May 30, 2024.

The Embrace Wellness Portal will continue to be active, and your visit notes will remain accessible. Copies of your labs and imaging can be obtained from the lab or Imaging facility where the testing was performed LabCorp 800-845-6167, St. Luke’s 208-381-222, St. Al’s 208-367-2121, Interpath 208-375-2350, Treasure Valley Lab 208-367-6392; Intermountain Medical Imaging 208-954-8130.

If you have any other questions or how to settle your account balance with Embrace Wellness, please feel free to contact my Practice Administrator, Amy Dusseau, at 208-287-8402. If you have questions about FMI/COH and what insurance plans they participate with, please contact FMI directly at 208-385-7711. Embrace Wellness staff cannot speak to FMI/COH policies.
